Position Title: Registered Nurse / Clinical Nurse – Community Palliative Care

Position Responsible to: Clinical Coordinator

Frequency of Role Review: 3 Months Probationary then at least annually

Status: Level 1 or level 2 PP 1-8 RN Award 2020 (Relevant to recent clinical experience)
Min 48 hr / fortnight (with on-call and weekend component)


The Registered Nurse / Clinical Nurse is responsible for the delivery of palliative care and cancer support services to clients and families within the Cooloola Sunshine Coast area, both within the home of the client, the Centre and other service provider venues.


Little Haven Palliative Care provides compassionate, caring and professional palliative care to incurably ill individuals, their families and friends in the place of their choice. Little Haven’s philosophy of care springs from a value base emphasising the value and dignity of all life, compassionate service, empowerment and shared responsibility for each other’s wellbeing.


Little Haven Palliative Care Inc is a registered Not-For-Profit Charity and has Public Benevolent Institution status. It is governed by a Board of Directors. The Business Manager of Little Haven is responsible for the overall management of the Service and reports to the Board.

Little Haven Palliative Care is a specialist community based palliative care service operating in the Sunshine Coast area of Queensland. The service seeks to improve the ability of our community to support people facing terminal illness, grief and bereavement. It includes a specialist home care team that offers support to individuals facing terminal illness and their families supporting people’s choice to die in their own home with their loved ones around them and optimising quality of life. It also includes bereavement support and enhancing community capacity through education about palliative care, death, and dying, and loss and grief.

Little Haven is a dedicated to providing relief from pain and suffering and is a member of Palliative Care Qld and Palliative Care Australia.


The position of  Registered Nurse / Clinical Nurse is based in the Little Haven Palliative Care office. The office is conducive to a relaxed, informal working environment for staff and volunteers. Little Haven is committed to creating a supportive, inclusive and participative working environment for staff.



  • Provision of palliative care and cancer support services, including nursing, counselling and support to palliative care clients and their families and carers, within the home of the client, or local hospitals and nursing homes.


  • Assess client care needs, make decisions on clinical palliative care in the home, and in conjunction with the team, coordinate client care, refer and liaise with GP, medical specialists, support services according to the policies set down by Community Health and Community Health Nursing Services.
  • Identify, limit, and report any safety and security risks, manual handling risks associated with care provided in the community and the clients home. Abide by the OH&S policies of SLHD, Community Health and Community Health Nursing.
  • Ensure all activities and functions associated with the role address specific requirements identified in the Quality Policy and Quality Objectives.


  • Participation in the development and delivery of an education program for volunteers, staff, palliative care clients and their families.


  • Liaison with all other service providers within the scope of cancer, terminal and palliative care, in order to provide quality accessible care for all members of the community requiring such services. Such providers include relaxation and complementary therapy practitioners, Community Health, Department of Veteran’s Affairs, occupational and physio therapists.


  • Utilisation of computer based technology to maintain client database utilising the PalCare System.


  • Case management of palliative care clients with complex care needs in conjunction with palliative care team.
  • Manage the changing day to day workload in a time efficient manner.
  • Provide a non-judgemental approach to individual client, cultural and environmental differences.
  • Working autonomously in a flexible working environment.
  • Function as a member of the team through active participation in team activities, discussions and the support of co-workers.


  • Performance evaluation will be undertaken at three months after commencement and then annually thereafter. A performance review may be conducted at any time at the direction of the Clinical Coordinator.


  • Unrestricted Drivers Licence (Qld)
  • Good oral and written communication skills
  • Recent post graduate nursing experience / qualifications (can be working towards) or equivalent clinical experience working in palliative care field (preferred).
  • Ability to work independently and in consultation with other team members
  • Maintain Currency of nursing registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Computer skills
  • Provide Current Positive Police Check
  • Provide current immunisation record


  • Work to the Terms and Conditions of the Registered Nurses Award
  • Agree to and sign Little Haven Palliative Care Confidentiality Agreement.
  • Agree to and sign Little Haven Palliative Care Code of Conduct Contract.
  • Work to and abide by the policies and procedures of Little Haven Palliative Care.
  • A probationary period of 3 months that includes a progress formative appraisal.
  • Participate in annual position formative appraisal.
