Little Haven currently has a position available for a Clinical Nurse with specialist palliative nursing experience wishing to join our highly regarded team.

Recent relevant Palliative Care / Community nursing experience is required, and a willingness to participate in the on-call roster is essential.

POSITION DESCRIPTION

Position Title: Clinical Nurse – Community based Palliative Care

Status: Level 2 PP 1-8 Registered Nurses Award with EBA
(Relevant to recent clinical experience)
Min 48 hr / fortnight (with some on-call and occassional weekend component)

The Clinical Nurse will join our team of nurses providing specialist home based palliative care visits to clients and families across the beautiful Gympie / Sunshine Coast region.

THE NATURE OF THIS ORGANISATION

Little Haven Palliative Care is a registered Not-For-Profit Charity specialising in community based palliative care, operating in the Gympie/Sunshine Coast area of Queensland. Little Haven provides 24 hour on call nursing care and support to the terminally ill and their family, specialised equipment loan, complementary therapies and bereavement support at no cost to the patient. This support provides palliative patients and their families the dignity, care, comfort and confidence to remain in the place of their choosing for their end of life.

Little Haven believes early access to support improves both quality and quantity of life and advance care planning.

Little Haven seeks to improve the ability of our community to support people facing terminal illness, grief and bereavement, enhancing community capacity through education about palliative care, death, and dying, loss and grief. A compassionate community model with a 40 + year history of supporting people at the right time in the right place.

WORK ENVIRONMENT

Little Haven is committed to creating a supportive, inclusive and enjoyable working environment for our team. Community visits are conducted using Little Haven’s fleet of vehicles from our base office in the grounds of Gympie General Hospital.

Key Position Responsibilities

  • Provision of palliative care services, including nursing, counselling and support to palliative care clients and their families and carers, within the patients home environment.
  • Demonstrated clinical proficiency in advanced palliative care, holistic assessment, symptom management, both pharmacological and non-pharmacological that is evidence-based and informed by clinical knowledge.
  • Assess client care needs, make decisions on clinical palliative care in the home, and in conjunction with the team, coordinate client care, refer and liaise with GP, medical specialists, and other Community Nursing Services.
  • Abide by the quality management system and policies and procedures of Little Haven Palliative Care Inc in line with the National Safety and Quality Health Standards v2.

TECHNOLOGY

  • Utilisation of computer based technology to maintain client database utilising the PalCare System. Clinical nurses are provided with a dedicated iPad and iPhone with patient care software installed.

SELECTION CRITERIA

  • Unrestricted Drivers Licence (Qld)
  • Good verbal, computer literacy and written communication skills
  • Recent post graduate nursing experience / qualifications (can be working towards) or equivalent clinical experience working in palliative care field (preferred).
  • Certificate of currency of Australian Health Practitioner Regulation Agency (AHPRA) registration
  • Provide Current Positive Police Check
  • Provide current immunisation record

TERMS AND CONDITIONS OF EMPLOYMENT

  • Work to the Terms and Conditions of the Registered Nurses Award with above award rates of pay offered
  • Agree to and sign Little Haven Palliative Care Code of Conduct and Confidentiality Agreement.
  • Work to and abide by the policies and procedures of Little Haven Palliative Care.
  • A probationary period of 3 months that includes a progress formative appraisal.
  • Base Hourly Rate above Award rates and conditions
  • Salary Sacrifice up to $600 per pay to a credit card, mortgage or nominated bill.
