Located in the beautiful Gympie Region, Little Haven provides high-quality community based specialist palliative care to patients across a 70km radius from our base in the Gympie Hospital grounds.
As a Clinical Coordinator, you will ensure our palliative patients, their family and clinical team are well supported. Combining case management, clinical governance, patient scheduling and staff management, the role of Clinical Coordinator is essential to ensure the leadership and coordination of all aspects of clinical care.

THE NATURE OF THIS ORGANISATION
Little Haven Palliative Care is a registered Not-For-Profit Charity specialising in community based palliative care, operating in the Gympie/Sunshine Coast area of Queensland. Little Haven provides 24 hour on call nursing care and support to the terminally ill and their family, specialised equipment loan, complementary therapies and bereavement support at no cost to the patient. This support provides palliative patients and their families the dignity, care, comfort and confidence to remain in the place of their choosing for their end of life. At Little Haven we believe early access to support improves both quality and quantity of life and advance care planning.
Little Haven seeks to improve the ability of our community to support people facing terminal illness, grief and bereavement, enhancing community capacity through education about palliative care, death, and dying, loss and grief. A compassionate community model with a 40 + year history of supporting people at the right time in the right place.
WORK ENVIRONMENT
The position of Clinical Nurse is based in the Little Haven Palliative Care office. Community visits are conducted using Little Haven’s fleet of vehicles from our base in the grounds of Gympie General Hospital. Little Haven provides a supportive, inclusive and participative working environment for staff.
Key Responsibilities
- Provide case management of palliative clients, clinical leadership and supervision of the Little Haven clinical team
- Manage the patient scheduler and day to day workload in a time efficient manner.
- Complete and send the Weekly case conference report / Attend the weekly Multi Disciplinary Team meeting
- Maintain scheduled imprest drug levels and compliance with Health Regulations General Approvals – Medicines
- Support Little Haven’s policies and procedure including risk management strategies and staff well being
- Promote a culture of service delivery with a focus on continual quality improvement
- Maintain information systems that supports and enhances the provision of accurate documentation
To be successful, you will have:
- Current Division 1 Registered Nurse registration with AHPRA
- Minimum 5+ years’ experience as a Clinical Nurse – Palliative Care
- Demonstrated experience in clinical governance, NSQHSS v2 accreditation
- Valid working rights in Australia (Passport, Birth Certificate etc)
- Current Unrestricted Qld Drivers Licence
- Provide Current Positive Police Check
- Provide current immunisation record
TERMS AND CONDITIONS OF EMPLOYMENT
- Position is permanent part time – 68hrs / fortnight, remunerated Level 3 PP 1 – 4 2022 RN Award (Min $52/hr)
- Work to the Terms and Conditions of the Registered Nurses Award 2022 with above award rates of pay offered
- Work to and abide by the policies and procedures of Little Haven Palliative Care
- A probationary period of 3 months that includes a progress formative appraisal
- Agree to participate in the Little Haven On Call roster as needed (max. 1 shift / fortnight)
- Provision of 2 current industry specific referee details
